In consideration of my engagement as a model, upon the terms within stated, I hereby give Jono Photography, his heirs, legal representatives and assigns, those from whom Jono Photography is acting, and those acting with his/her/they authority and permission:

 

I give the unrestricted right and permission to use, re-use, publish, republish, produce and vend. The released photographic images, video content captured of me or in which I may be included in tact or in part, composite or distorted in character or form, including reasonable changes, transformations or reproduction in all media, print or in web.

 

I am granted authorized rights to use these images for purposes of self promotion.

 

I hereby release all rights and holdings to Jono Photography. I acknowledge and agree that this release and agreement to hold harmless shall continue indefinitely. 

 

I hear by a firm that I am over the age of majority and have the right to legalize this contract in my own name or stage name. I have read the above authorization, release an agreement, prior to its execution; I fully understand and give my full consent to all content created.

 

This agreement shall be binding upon me and my heirs, legal representatives and assigns.
